The US Food and Drug Administration has opened two new foodborne illness investigations covering 65 reported cases, while stressing that no product has yet been identified in either outbreak. The agency added the investigations to its active outbreak table on September 23 and has begun traceback work.

One investigation involves 25 cases of E. coli O157:H7 and carries FDA reference number 1421. The table also lists USDA Food Safety and Inspection Service reference 2026-04, indicating coordination across federal food safety jurisdictions as investigators work to identify the source.

The second investigation concerns 40 cases of Listeria monocytogenes under FDA reference number 1417. FDA has initiated traceback, but its public table does not identify a commodity, manufacturer, brand or distribution channel.

No recall is listed for either investigation. That distinction is important for retailers, foodservice operators and consumers because the case counts do not establish that any particular product is implicated. Naming a category or supplier before investigators publish evidence would create an unsupported commercial and public health claim.

FDA’s outbreak process combines epidemiological evidence, laboratory findings and supply chain records. The agency says it issues a public advisory when an investigation produces specific, actionable steps, and it may withhold a product name until sufficient evidence connects that product to illnesses.

For food businesses, the practical response is to keep traceability records and supplier contacts ready rather than make speculative withdrawals. Lot codes, receiving records, shipment destinations and customer notification procedures can become essential if FDA or USDA later identifies a product.

The same FDA update raised the case count in a separate Salmonella Newport investigation from 111 to 116 and said sampling had begun. That outbreak also remains without an identified product, illustrating how several investigations can be active at different stages without an immediate recall.

The next material developments would be identification of a food category, a recall, an inspection or sampling result, or a formal advisory from FDA, USDA or the Centers for Disease Control and Prevention. Until then, the verified position is limited but consequential: two active traceback investigations, 65 cases and no identified product.
